Health Program Requirements
As a provider of care, students have a legal and ethical responsibility to make certain their health status does not jeopardize the quality of patient care. For this reason, every student in health science must meet Physical, TB testing, and Immunization Requirements appropriate to their specific program. Please print the correct form below and take to your healthcare practitioner when completing the requirements.
Health Science Programs: Dental programs (DA, DH, DT), Electroneurodiagnostic Technology (END), Health Information Technology programs (HIT, MC, MT), Medical Assisting (MA), Nursing, Occupational Therapy Assistant (OTA), Physical Therapy Assistant (PTA), Respiratory Therapy Assistant (RTA), and Surgical Technology (ST). High School Academy: Continuing Education Programs:
All required health information will be kept on record at the Campus Health office. All records are confidential, kept for 10 years post-graduation, and stored in a locked office. Keep a copy of all your health records. We are not responsible for lost or destroyed documents. You will need these records sometime in the future for a job, schooling or for medical reasons. |
